Who we are

Premier is a social services agency committed to delivering equitable, person-centered, and culturally responsive case management services throughout Minnesota. We specialize in coordinating supports for aging adults, individuals with disabilities, those with complex medical or mental health needs, individuals with brain injuries, and others who require long-term care planning. Our role is to partner with individuals and their families to develop person-centered service plans and coordinate access to the providers, supports, and community resources that best meet their needs. We do not deliver direct care services—we ensure the right ones are in place.

Premier was founded to close critical service gaps experienced by historically underserved populations, including immigrants, refugees, and individuals navigating cultural, linguistic, and systemic barriers. Grounded in core principles of informed choice, self-direction, and cultural responsiveness, our case managers advocate for each client’s goals, preferences, and strengths. Through collaborative planning, ongoing monitoring, and cross-system coordination, we help ensure that individuals can live safely, independently, and with dignity in the settings they choose.

Mission

Our mission is to coordinate meaningful, person-centered services that promote self-reliance, independence, and community inclusion for individuals with disabilities and others facing complex challenges. We do this by partnering with individuals and families to develop care plans, navigate service systems, and connect with trusted providers who support their goals. Guided by cultural responsiveness, integrity, and a commitment to equity, Premier delivers every service with dignity, purpose, and the belief that every person deserves a pathway to a stable and fulfilling life.
