Waiver Case
Management
This is a service that helps individuals eligible for Minnesota’s Medicaid waiver programs access and coordinate the long-term supports they need to live in the community, rather than in institutional settings. Case managers work with individuals and their families to assess needs, develop service plans, coordinate with providers, and ensure services reflect the person’s goals, preferences, and cultural background.

Community Access for Disability Inclusion (CADI) - For individuals with physical disabilities or chronic medical conditions who need services to live independently and avoid institutional care.
Developmental Disabilities (DD) Waiver - For individuals with intellectual or developmental disabilities who need long-term services to support daily living, skill development, and community engagement.
Brain Injury (BI) Waiver - For individuals with acquired or traumatic brain injuries who require support with cognitive, behavioral, and physical functioning in community settings.
Alternative Care (AC) Program - For adults age 65+ who are not on Medical Assistance but need help remaining safely in their homes and delaying institutional placement.
Elderly Waiver (EW) - For adults age 65+ who qualify for Medical Assistance and require support services to age in place in their communities.
Community Alternative Care (CAC) Waiver - Serves individuals with chronic illnesses who require hospital-level care, enabling them to receive services in community settings.
Rule 185 Case
Management
Serves individuals with developmental disabilities or related conditions by providing long-term case management focused on planning, service coordination, and ongoing support. This service helps connect individuals to medical, social, educational, and other supports needed to promote independence and reduce the impact of their disability.
Audit & Compliance
Consulting Services
